The upper back is the area patients mention last and are bothered by most. The rolls that gather under the bra strap and at the side of the chest are visible in every fitted top and every photograph from behind, and they are made of loose skin, not fat, which is why diet and training do not move them. A bra line back lift removes those rolls through a single horizontal incision hidden under the bra, and it is one of the least discussed and most satisfying operations in body contouring after weight loss.
What a Back Lift Treats
Skin on the back is thick and tends to fold rather than hang. After weight loss, or simply with age in some body types, the skin between the shoulder blades and the lower ribs folds into one or more horizontal rolls that continue around the side of the chest toward the breast. Liposuction can reduce the fat inside a roll but does nothing for the skin, and in a loose roll it can leave the fold emptier and more visible. The only way to remove a roll of skin is to remove the skin, and the back lift does that in a way that hides the scar where a bra already sits.
The Bra Line Back Lift
The incision runs horizontally across the back at the level of the bra band, extending as far around the sides as the excess requires, and its position is drawn with the patient standing, in her own bra, so that the final scar sits under the band. The rolls above and below the line are lifted and the excess skin between is removed as a long ellipse, with liposuction of the surrounding area to blend the transition. The two edges are brought together and closed in layers, with the deep layer taking the tension so that the scar stays fine. Drains are used when the area is large and come out at one to two days; smaller lifts often need none.
The back lift is not one of the major lifts, so it combines easily. The commonest pairing is with breast surgery, because the roll at the side of the chest is continuous with the outer breast and treating one without the other leaves a step. A breast reduction or breast lift with a bra line back lift, done in the same position and the same session, reshapes the whole upper trunk. It also pairs with an arm lift when the excess runs from the arm into the side of the chest.
Who Is a Good Candidate
A good candidate has one or more rolls of loose skin across the upper back or the sides of the chest, is at a stable weight, does not smoke or is prepared to stop around surgery, and accepts a horizontal scar under the bra line in exchange for a smooth back. Women after major weight loss are the largest group; men with loose skin across the back after weight loss are candidates too, and in men the scar is placed at the level where a T-shirt seam would sit. Patients whose problem is fat rather than a fold do better with liposuction alone, and I say so rather than adding a scar they do not need.
The Operation
A back lift is performed under general anesthesia, lying face down, and takes about two hours on its own. Combined with breast surgery the session runs longer and you are turned during the operation. Liposuction of the flanks and the area around the incision comes first when it is needed. The ellipse of skin is removed, the deep layer is sutured to take the tension, the skin is closed with absorbable sutures, and a compression garment or a supportive bra goes on before you wake. You stay one night in hospital, two when the back lift is combined with other lifts.
Your Days in Istanbul
Day 0: surgery and one night in hospital. Days 1 to 3: at your hotel in the garment; the back feels tight, particularly when reaching forward or lifting the arms, and this eases over the first week. Drains, if placed, come out at day one or two. Day 4 or 5: final examination, fresh dressings, and you fly home. Five days in Istanbul is enough for a back lift on its own or with breast surgery. Desk work within a week, walking as exercise from the first days, lifting and gym at about six weeks. The garment or supportive bra is worn for six weeks. The scar is red for the first months and fades over a year; under a bra band it is invisible, and in a swimsuit it sits where most straps run.

Risks and Honest Limits
The specific risks are a scar that widens, particularly at the sides where the skin moves most; fluid collection under the skin after the drains come out; delayed healing along part of the incision, which heals with dressings; temporary numbness of the skin around the scar; and asymmetry of the two sides that may need a small revision. The back is well supplied with blood and heals reliably, and problems are less frequent than in the thigh or arm.
The limits: a back lift removes the rolls of the upper back and sides; it does not treat the lower back and flanks, which belong to a tummy tuck or a lower body lift, and it does not change the shape of the shoulder blades or the muscle underneath. The scar is permanent and horizontal; it hides under a bra and most tops, and it shows in a backless dress. If you gain weight afterward, the back changes with the rest of you.
Cost of a Back Lift in Turkey
A back lift in Istanbul costs considerably less than the same operation in the United Kingdom, Germany or the wider EU, and the difference comes from local operating costs rather than the technique. The figure depends on how far around the sides the incision runs, on the liposuction added, on whether breast surgery is done in the same session, and on the operating and hospital time involved.
