Many patients who ask me about eyelid surgery do not have an eyelid problem. Their upper lids look heavy because the brow above them has come down, and if I remove eyelid skin without lifting the brow, the lid looks better for a while and the brow drops further to fill the space. A brow lift puts the eyebrow back where it belongs and, in doing so, opens the eye. The decision that matters is which kind of brow lift, because a brow that has dropped at the outer corner needs a different operation from one that has dropped across its whole width.
Is It the Brow or the Eyelid?
The test is simple and you can do it in a mirror. Lift the eyebrow gently with a finger to where it sat ten years ago. If the heaviness of the upper lid disappears, the brow is the problem. If a fold of skin still hangs over the lashes, the eyelid is the problem, and if both improve you probably need both. Most patients over fifty fall into the third group, which is why a brow lift is so often paired with upper blepharoplasty in a single session.
The second question is where the brow has dropped. A brow that has fallen only at the tail, giving a tired or sad look with hooding at the outer corner of the eye, needs the outer third lifted and nothing else. A brow that has flattened across its whole length, with deep horizontal forehead lines from years of holding it up, needs the whole forehead released and repositioned. Lifting the whole brow when only the tail has dropped produces the surprised look that gave brow lifts a bad name. Lifting only the tail when the whole brow has dropped does too little.
The Techniques and How I Choose
I use all of the standard approaches and decide by the pattern of descent, the height of the hairline, the depth of the forehead lines and the patient’s hair.
Endoscopic brow lift
Through four or five small incisions hidden in the hair, a camera and fine instruments release the forehead tissue from the bone and the muscles that pull the brow down, the tissue is lifted and fixed at its new height. There is no long scar and no skin is removed, so the forehead is not shortened. It suits patients with a normal or low hairline, moderate brow descent across the whole width, and mild to moderate skin excess. It is the technique I use most.
Temporal lift
A temporal lift, sometimes called a temple lift or lateral brow lift, raises the outer third of the brow only. I perform it endoscopically through a small incision in the temple hair, releasing the tissue over the outer brow and fixing it higher and slightly outward. It is the right operation for the patient whose problem is hooding at the outer corner with a brow that is otherwise in place, and it is often the one piece that makes an upper eyelid operation look complete. It is quick, the recovery is light, and there is nothing to see in the hairline afterward.
Hairline (pretrichial) brow lift
When the forehead is high, lifting the brow endoscopically would raise the hairline further. The hairline approach places the incision along the front edge of the hair, removes a strip of forehead skin, and can lower the hairline at the same time as lifting the brow. It is the technique for the patient with a tall forehead and a heavy brow; the scar sits at the hairline and hair grows through it. When the forehead height is the main complaint, this becomes a forehead reduction rather than a brow lift.
Direct and mid-forehead lifts
In older men with deep forehead creases and thinning hair, the incision can sit inside an existing crease directly above the brow or across the forehead. The lift is precise and strong; the trade is a visible fine scar, which is acceptable in a deeply lined forehead and not in a smooth one. I reserve it for that specific patient.
What About Injections and Threads?
Wrinkle-relaxing injections can soften the muscles that pull the brow down and give a millimeter or two of lift for a few months. For a brow that has genuinely descended they do not work, and patients who have tried them for years usually arrive knowing that. Thread lifts of the brow are not lasting; the threads dissolve or cut through, and the brow returns to where gravity put it. I do not offer them for the brow, for the same reason I do not offer them for the outer corner of the eye.
Who Is a Good Candidate
A good candidate has a brow that sits at or below the bony rim of the eye socket, hooding at the outer eye, or a tired, heavy look that lifts with a finger; is in good general health; and understands that a brow lift is about position, not about erasing every forehead line. Age is not the criterion; some patients need a temporal lift in their late thirties, others do not need any brow surgery at seventy. Patients with a very high hairline are steered toward the hairline approach; patients with advanced male pattern hair loss are told frankly that hiding endoscopic scars is harder and that the direct approach may be the better trade.
The Operation
A brow lift is performed under general anesthesia, or under sedation for an isolated temporal lift, and takes one to two hours on its own. Combined with eyelid surgery or a facelift the session runs longer, and I combine facial procedures freely when the anatomy calls for it; brow lift, eyelid surgery, facelift, neck lift and fat grafting can all be done in one session. Fixation of the lifted tissue is achieved with absorbable devices or sutures anchored to the bone, so nothing needs removing later. The incisions are closed with absorbable sutures; there are no sutures or staples to take out. You stay one night in hospital.
Your Days in Istanbul
Day 0: surgery and one night in hospital. Days 1 to 3: at your hotel; the forehead feels tight and the scalp numb, swelling drifts down into the eyelids and settles by the end of the first week, and bruising is mild. Sleep with the head raised. Day 3: final check, and you can fly home. Three days is enough for an isolated brow lift because there is nothing to remove and no drains; when the brow lift is part of a facelift, the facelift schedule of five to seven days applies. Desk work is comfortable within a week, exercise at three to four weeks. The brow position looks slightly high in the first weeks and settles into its final place over two to three months; the numbness of the scalp behind the incisions can take longer to resolve.

Risks and Honest Limits
The specific risks are temporary or, rarely, lasting numbness of the scalp behind the incisions, thinning of hair around the incisions for a few months, asymmetry of brow height that may need adjustment, and, very rarely, weakness of the forehead muscle on one side from bruising of the nerve, which almost always recovers. Bleeding under the scalp is uncommon. A brow that is lifted too high looks worse than one left alone, which is why I set the outer brow a little higher than the inner and stop before the eyebrow leaves its natural arc.
The limits: a brow lift does not remove upper eyelid skin, so hooding from true skin excess needs an eyelid operation as well; it does not erase deep static forehead lines, although releasing the muscles softens them; and it cannot correct asymmetry that comes from the underlying bone. The result lasts many years, but the brow continues to age from its new position.
Cost of a Brow Lift in Turkey
A brow lift in Istanbul costs considerably less than the same operation in the United Kingdom, Germany or the wider EU, and the difference comes from local operating costs rather than the technique. The figure depends on the approach, on whether it is an isolated temporal lift or a full endoscopic lift, and on whether eyelid surgery or a facelift is done in the same session. Will a brow lift make me look surprised?
