FUPA Surgery: Liposuction, Mons Lift, Tummy Tuck or Panniculectomy

FUPA is the informal name for fullness of the area above the pubic bone, the mons pubis. It covers very different problems: a cushion of fat in a slim woman, a bulge above a cesarean scar, a pubic area that has dropped after large weight loss, a fold of abdominal skin hanging over it, or a fat pad in a man that hides part of the penis. Each is treated differently, and the wrong operation is the most common reason for a disappointing result.

This page explains how I tell fat from loose skin from an overhang, which operation fits each, how a panniculectomy differs from a tummy tuck, and what changes in men and after weight loss with GLP-1 weight-loss medication or bariatric surgery.

FUPA, Mons Pubis, Pannus: What It Is and What It Is Not

The words patients search for describe different layers, and the layer decides the operation:

  • FUPA, pubic fat. Fat stored in the mons pubis, the cushion over the pubic bone. In many people it is simply where the body keeps fat, and it can stay after weight loss elsewhere.
  • Mons ptosis, pubic sag. The skin and fat of the mons have descended, so the pubic area sits lower and, in women, can pull on the upper labia. This is a skin and support problem, not only a fat problem.
  • Pannus, apron, overhang. A fold of abdominal skin and fat that hangs over the pubic area. It is not part of the mons, but it is often what people mean by FUPA.
  • Monsplasty, mons lift, pubic lift. An operation that reduces the fat of the mons and lifts its skin to a higher, flatter position, either as part of a tummy tuck or as an operation on its own.
  • Panniculectomy. Removal of the hanging apron only, without muscle repair, navel reshaping or contouring of the upper abdomen.
  • Pubic liposuction, mons liposuction. Removal of fat from the mons through small incisions, without removing skin.
  • Escutcheon, suprapubic fat pad. The fat pad above the penis in men. When it is large, the penis looks shorter or is partly hidden.

FUPA is not a medical diagnosis. It becomes a medical problem when an overhang causes rashes, infection or hygiene difficulties, or interferes with urination or sexual function.

Fat, Skin or Overhang: The Three Types

I assess the area in three layers, standing and lying down, because each layer has its own answer.

Fat with firm skin. The mons is full but sits in the right place, and the skin springs back when pinched. This is common in younger women and in men, and the fat can be reduced without removing skin.

Descent of the mons. The skin of the pubic area has stretched and the mons has moved down. This follows large weight loss and several pregnancies, and a 2026 review described how a healing cesarean scar can pull the mons downward. A published classification grades the mons by the amount of fat and the degree of descent, and matches the treatment to both. When the mons is very lax, removing the fat alone makes no sense to me: a lift is needed as well. This is decided patient by patient.

An overhang from above. The pubic area may be normal, but a fold of abdominal skin hangs over it. Here the operation is on the abdomen: usually a tummy tuck, which corrects the mons in the same session, or a panniculectomy when the only goal is to remove the overhang.

When Surgery Helps and When It Does Not

Weight loss reduces the fat of the mons, and for some people that is all that is needed. The mons is also one of the places where fat stays longest, and exercise cannot target it. No device, cream or garment takes up skin that has dropped or an apron that hangs. In men there is a further limit: much of the fullness of the lower abdomen can be fat inside the abdomen, around the organs, which no body contouring operation can reach.

Surgery makes sense when the shape bothers you, your weight has been stable for four to six months, and what remains is fat with firm skin, a dropped mons or an overhang. If you are still losing weight, wait.

Panniculectomy vs Tummy Tuck

A panniculectomy removes the hanging apron of skin and fat and nothing else. It is a functional operation for a patient whose overhang causes rashes, infections or hygiene problems, and in some countries it is covered by insurance for that reason. I perform it as a separate operation when that is the goal.

A tummy tuck does everything a panniculectomy does and then rebuilds the shape: separated muscles are repaired where needed, the navel is reshaped, the waist is contoured with liposuction, and the pubic area is lifted and tightened with the rest of the lower abdomen. The details are on the tummy tuck page. Almost every patient who asks me about a panniculectomy is better served by a tummy tuck.

What matters more than the name of the operation is weight and health. In a 2025 analysis of 18,891 patients in a large American surgical database who had a panniculectomy with or without a tummy tuck, 11.3 percent had a complication within 30 days: 15.2 percent of patients with obesity and 7.3 percent of those without.

The Options for a FUPA

Liposuction of the Mons

For fat with firm skin and a mons that sits in the right place. I perform it on its own, without a tummy tuck, and the fat is removed through small incisions. If the lower abdomen and flanks are also full, I treat them in the same session so that the contour is even. I use VASER and MicroAire liposuction, which make dense, fibrous fat easier to treat. Liposuction does not lift the mons: if the skin has dropped, removing the fat under it can make the sag more visible.

Mons Lift (Monsplasty), Alone or with a Tummy Tuck

For a mons that has dropped, with or without an overhang above it. When only the pubic area has dropped and the abdomen above is firm, I perform a mons lift on its own, without a tummy tuck. When the abdomen is also loose, the pubic area is lifted and tightened with the rest of the lower abdomen through the same low incision, and its fat is reduced with liposuction in the same session; within a tummy tuck, it is not a separate operation. Published series report improvements in hygiene, sexual function and body image after monsplasty, and a comparison of 60 women after weight loss found a more stable result when the mons was corrected during the tummy tuck than with a tummy tuck alone.

After Major Weight Loss

After large weight loss the overhang often continues around the hips, and the operation becomes an extended or Fleur-de-Lys tummy tuck or a lower body lift, with the mons corrected in the same session. The body contouring after weight loss guide explains how these fit together.

FUPA in Men

When the fullness is a fat pad above the penis with firm skin, liposuction of the mons flattens the area and makes more of the penis visible, which many men notice as a change in length. A man usually does not need muscle repair, and when the skin is loose, the male tummy tuck places the scar lower and flatter, under the waistband. I have written about the differences in the male body after weight loss.

When the fat pad and skin are so large that the penis is buried, urination, hygiene and sex are affected. Buried penis repair is a reconstructive operation that often includes removal of the fat pad; published series show that it succeeds in most men, but wound complications are common at a high body weight. This correction may need to be done together with a urologist, and when I think a urology consultation makes sense, I ask for one.

After GLP-1 Weight-Loss Medication or Bariatric Surgery

Before surgery after large weight loss:

  • A stable weight for four to six months.
  • Keep taking your GLP-1 medication. Tell us the dose and your last injection date; instead of stopping it, you take clear liquids only for the 24 hours before surgery.
  • Blood results first. After bariatric surgery, protein, iron, vitamin B12 and vitamin D are checked and corrected, because long incisions heal well only in a well-nourished body.

A 2025 analysis of more than 24,000 panniculectomies, comparing about 6,000 patients who had used GLP-1 medication with about 4,100 who had bariatric surgery, found fewer skin infections, fluid collections and bleeding complications in the medication group.

How I Perform It

Every FUPA correction is performed in an accredited hospital under general anesthesia, with an anesthesiologist present; mons liposuction alone can also be done under sedation. The incision is drawn with you standing, low inside the underwear line. Liposuction is done in one session, never staged. Every patient wears compression stockings and calf compression during surgery, walks on the day of surgery, and receives clot prevention medication when it is indicated. A FUPA correction can be combined with other body surgery within my rule of no more than three major lifts in one operation, and genital procedures such as labiaplasty, described on the genital aesthetics page, can join the same session.

Your Week in Istanbul

  • Day 0: surgery. One night in hospital after liposuction or a mons lift on its own, or mons liposuction with labiaplasty; mons liposuction under sedation can be a day case. Two nights after a tummy tuck or a larger combined plan. You walk on the day of surgery.
  • Days 1 to 5: at your hotel, in a compression garment, walking several times a day. After a tummy tuck, drains come out gradually, usually around the third day. Two to three lymphatic massage sessions are done in Istanbul when liposuction is part of the plan.
  • Day 5 to 7: final check and dressing the day before departure, and you fly home in your compression garment. Seven days cover a tummy tuck or a large combined plan.

Recovery

Swelling of the pubic area can extend to the labia or the scrotum in the first two weeks before it settles. You may shower five to seven days after the drains are removed. The compression garment is worn for six to eight weeks: day and night for the first three weeks, with one to two hours a day off, and after day 21 optional during the day but still worn at night.

After liposuction most people return to desk work within one to two weeks; after a tummy tuck, after about two weeks, with light cardio from week six and abdominal training from week eight, as described in tummy tuck recovery time. Most swelling settles over three to four months, and the shape of the mons is judged at six months.

Risks and Honest Limits

The risks to plan against are fluid collection, delayed healing along the scar, infection, temporary numbness, asymmetry and, rarely, blood clots. They are lower at a stable, moderate weight and in non-smokers, and higher with obesity, diabetes and after very large weight loss. If you smoke, read my guide to smoking and surgery first.

Three honest limits. A mons lift always leaves a scar; it is placed low and fades, but it does not disappear. Liposuction cannot lift skin, and when the mons is very lax, it needs a lift as well as fat removal. And surgery does not change where your body stores fat: if your weight rises again, fat can return to the mons.

FUPA Surgery Cost in Turkey: What Shapes the Price and What Is Included

I do not publish a single price, because liposuction of the mons and a tummy tuck with a mons lift after weight loss are not the same operation. What moves the figure:

  • The operation. Liposuction alone, a mons lift alone, a panniculectomy, a tummy tuck with a mons lift, or an extended operation after weight loss.
  • Muscle repair. Needed in many women after pregnancy, rarely in men.
  • What is added in the same session. Liposuction of other areas, breast surgery, an arm or thigh lift, or genital procedures.
  • The hospital stay. Up to two nights, set by the extent of the operation.

I give a written quotation after reviewing your photographs and history. Every quotation is all inclusive: the surgery, the hospital stay, the anesthesia, medication, the compression garment, your accommodation, airport and clinic transfers, and the follow-up visits and post-operative contact in Istanbul.

Before and After: How to Read the Photographs

Compare the front and both sides, standing relaxed, in the same underwear and light. The side view shows a full or dropped mons. After surgery the mons should sit naturally above the pubic bone, not pulled up, and the honest photograph is at six months or later.

Results are in the before and after gallery. Many patients prefer not to have photographs of this area published, so in a video consultation I can show you more cases with a starting point similar to yours.

Send your photographs for an assessment. Photographs of your lower abdomen and pubic area from the front and both sides, standing relaxed in underwear, and one from the side while sitting, plus your height, current and highest weight, how you lost weight if you did, any pregnancies or cesareans, and any weight-loss medication you take. Send them on WhatsApp, +90 544 772 0772, or by email, info@mustafaaydinol.com, whichever you prefer.

Frequently Asked Questions

What is a FUPA?

An informal name for fullness above the pubic bone, the mons pubis. It can be fat, a dropped mons, or a fold of abdominal skin hanging over it, and each is treated differently.

Can you get rid of a FUPA without surgery?

Weight loss reduces the fat, and for some people that is enough. It does not lift a dropped mons or remove an apron of skin, and exercise cannot target fat in one area.

Should I have a panniculectomy or a tummy tuck?

A panniculectomy removes the hanging apron only. A tummy tuck also repairs the muscles where needed, reshapes the navel and contours the waist. I perform a panniculectomy as a separate operation when removing the overhang is the goal, but almost every patient who asks me about one is better served by a tummy tuck.

Can a FUPA be treated without a tummy tuck?

Yes. When the skin is firm, I perform liposuction of the mons on its own. When only the mons has dropped and the abdomen above is firm, I perform a mons lift on its own. A tummy tuck is needed when the abdomen itself is loose.

Can men have FUPA surgery?

Yes. The fat pad above the penis is often treated with liposuction, and loose skin with a male tummy tuck. A buried penis may need to be corrected together with a urologist. Fat inside the abdomen cannot be removed by any body contouring operation.

How long after weight loss can I have FUPA surgery?

When your weight has been stable for four to six months, whether you lost it with GLP-1 medication, bariatric surgery or diet and exercise.
