Thigh Lift in Turkey: The L and T Incisions, Liposuction and Recovery

The inner thigh is the last place loose skin is forgiven and the first place it is felt. Skin that rubs when you walk, that hangs when you sit, that has not changed with any amount of training or weight loss: this is what a thigh lift is for. Like an arm lift, it is an operation about a scar, and the shape of that scar is the whole conversation. I do most thigh lifts with an L-shaped or a T-shaped incision, I add liposuction to almost all of them, and I will tell you before surgery exactly where the line will run.

Why the Thigh Loosens and Why Only Surgery Fixes It

Inner thigh skin is thin, has little structural support, and stretches easily. After significant weight loss, whether through diet, surgery or weight-loss medication, it does not shrink back; the fat that filled it has gone and the envelope remains. With age the same skin loses its recoil and slides downward. Exercise builds the muscle underneath and does nothing for the envelope. Liposuction removes fat and, in thin, loose skin, can make the sagging worse. Energy-based tightening gives a modest change in mild cases. When skin hangs, the only treatment that works is to remove it, and that is a thigh lift.

The Incisions: Horizontal, L, or T

The pattern of the incision follows the pattern of the excess.

Horizontal (groin) incision. A scar hidden in the crease between the thigh and the groin, lifting the inner thigh upward. It suits mild laxity concentrated in the upper inner thigh, and its weakness is that it lifts vertically only; skin that is loose down the length of the thigh is not addressed, and a scar under tension in the groin can migrate downward.

L-shaped incision. A short horizontal segment in the groin crease plus a vertical segment running down the inner thigh. The vertical part removes the circumferential excess, which is what most patients after weight loss actually have, and the short groin segment lifts the upper thigh. This is the pattern I use most: the scar is longer, but it sits on the inner surface where the thighs meet, and it treats the whole length of the thigh rather than the top of it.

T-shaped incision. A full horizontal groin incision joined to a vertical one, for patients with a large amount of excess in both directions, typically after massive weight loss. The trade is a longer scar for a thigh that is genuinely reshaped rather than just tightened.

I do not choose the pattern to keep the scar short at the expense of the result. A horizontal-only lift on a thigh that needs a vertical one leaves loose skin below the knee of the scar and a patient who has paid for a scar without getting a thigh. The choice is made from your photographs and, at consultation, from where the skin gathers when it is lifted by hand.

Liposuction Is Part of the Operation

Liposuction is included in nearly every thigh lift I do. It thins the fat layer so that the skin can be redraped without bulk, it treats the outer thigh and knee area that the incision does not reach, and it lets the skin removal be more conservative because the tissue underneath is slimmer. I use ultrasound-assisted liposuction when the tissue is fibrous or the volume is large, but it is not a requirement; the technique is chosen for the thigh in front of me, not from a menu.

Who Is a Good Candidate

A good candidate has loose inner thigh skin that hangs or rubs, has been at a stable weight for several months, does not smoke or is prepared to stop around surgery, and is willing to accept a permanent scar on the inner thigh in exchange for a firmer contour. Patients after major weight loss are the largest group. A thigh lift counts as one of the major lifts, and I perform no more than three major lifts in one session, so a thigh lift can be combined with a tummy tuck, a buttock lift or an arm lift but not with everything at once; the combination planner shows what fits together and in what order.

Patients who are not yet candidates are those whose problem is fat rather than skin, for whom liposuction alone is the better operation, and those still losing weight, for whom the result would loosen again.

The Operation

A thigh lift is performed under general anesthesia and takes two to three hours for both thighs, longer when combined with other procedures. Liposuction comes first. The incision is then made along the planned L or T, the excess skin and fat are removed, the deep tissue is anchored to the firm fascia of the groin so that the weight of the thigh does not pull on the scar, and the skin is closed in layers with absorbable sutures. Drains are placed and come out at two to three days. A compression garment goes on in theatre. You stay one night in hospital, two nights when the thigh lift is combined with a tummy tuck or other major lifts.

Your Days in Istanbul

Day 0: surgery and one night in hospital; you stand and walk the same evening. Days 1 to 3: at your hotel in the compression garment, walking short distances several times a day, sleeping with the legs slightly raised. Drains out at day two or three. Day 4 or 5: final examination, a fresh dressing, and you fly home. Five days in Istanbul is enough for a thigh lift on its own; combined cases stay up to seven. Showering waits until the drain holes have closed. Desk work at one to two weeks, walking as exercise from the first week, gym at about six weeks. The compression garment is worn for six to eight weeks, with the daytime hours reduced as you heal and nights kept until the end. The scar is red for the first months and matures over twelve to eighteen months; the inner thigh position keeps it out of sight in normal clothing and in most swimwear.

Risks and Honest Limits

Thigh lifts have a higher rate of wound healing problems than most body contouring operations, because the inner thigh is a moist, mobile area with skin closed under some tension. Small areas of delayed healing along the scar are not unusual and heal with dressings; that is the main reason I keep tension low and anchor the deep tissue rather than the skin. Other specific risks are fluid collection after the drains come out, swelling of the lower leg that can take weeks to settle, numbness of the inner thigh that usually recovers, widening or downward migration of the groin scar, and asymmetry. Published series report that revision of the scar is the commonest reason for a second procedure. Blood clots are a risk in all body surgery and are managed with early walking, compression and, where indicated, anticoagulant injections.

The limits: a thigh lift firms and reshapes; it does not produce an athletic thigh, and it does not treat cellulite, which sits in the skin structure itself. If you gain weight afterward, the thigh changes with the rest of you. And the scar is permanent. It fades, it hides, but it is the price of the operation and it should be the thing you weigh most carefully before booking.

Cost of a Thigh Lift in Turkey

A thigh lift in Istanbul costs considerably less than the same operation in the United Kingdom, Germany or the wider EU, and the difference comes from local operating costs rather than the technique. The figure depends on the incision pattern, the amount of liposuction, whether other lifts are done in the same session, and the operating and hospital time involved.
